  • Does not guarantee that you will qualify for surgery
  • May use automated processing to evaluate your responses
  • May result in your information being shared with partner providers if you qualify

10.2 Multiple Assessment Tools

Our platform may include various assessment tools with different purposes, including but not limited to:

  • Primary qualification quizzes that determine surgery eligibility
  • BMI calculators
  • Insurance coverage assessments
  • Health risk evaluations
  • General educational assessments

Each quiz may collect different types of information as outlined in our Privacy Policy.

10.3 Accuracy of Responses

You agree to provide truthful and accurate responses to all quiz questions across any assessment tools we offer. Providing false information may result in inappropriate recommendations and provider matches.

11. Provider Matching

11.1 Matching Process

If you meet the qualification criteria based on your quiz responses, we may connect you with bariatric surgery providers in your area. This process:

  • Is based on your location, insurance information, and qualification status
  • Does not guarantee acceptance by the provider for surgery
  • Is not an endorsement of any specific provider's quality or services

11.2 Provider Relationships

WeightJourneyMD is not responsible for:

  • The quality or accuracy of information provided by matched providers
  • The outcome of any consultation or procedure with a matched provider
  • Any disputes that may arise between you and a provider

12. Payments and Fees

12.1 User Fees

The use of our qualification quiz and educational resources is currently provided free of charge to users. We reserve the right to change this policy in the future.

12.2 Provider Fees

Our business model involves charging healthcare providers for qualified leads. You acknowledge that providers may pay us when you attend a consultation resulting from our matching service.

13. Third-Party Websites and Content

The Site may contain links to third-party websites and services. We provide these links as a convenience and do not control, endorse, or assume responsibility for any third-party websites linked from our Site. You acknowledge and agree that WeightJourneyMD shall not be responsible or liable for any damage or loss caused by your use of any such website or service.
